When you arrive for your appointment, please make sure you have the following:

• Your insurance card. We will need to make a copy of it for our records.
  • Also, make sure you let us know if that information ever changes.
     
• A picture ID – your driver’s license is fine. We want to make sure that someone else is not using your stolen identity – unfortunately, we live in a world where this sort of thing happens, and drug-seekers will sometimes go to extremes to feed their habit! We will make a copy of this ID for your permanent record.
   
• Bring your completed new patient forms – they can be printed out from this page (click here, print, and complete). This will save all of us a lot of time once you arrive, if you have them filled out and ready for the front desk when you arrive.
   
• Please make sure you bring your co-payment each time you visit our office. This is required by your insurance company, and we always collect it at the time of service. Please review the “Insurance” section of our Web site for more information about our collection policies. We accept cash, check, Master Card, Visa, Discover, American Express, and even your debit card!
   
• Bring all medications that you are currently taking. This is an important part of your visit. Your physician will review these medications with you during your appointment.
   
• We need copies of any pertinent past medical records. You should try to bring copies with you when you come for your appointment – especially any immunization records.
   
• Please be on time. Allow for Charlotte traffic snarls. We make every effort to see you on time. Please review the “Appointments” section of our Web site for more information.
   
• Make sure you know how to get to our office. The section titled “Our Office” will allow you to print out directions from your home or office address.
